Depression Checklist

  • Persistent sad, anxious or empty feelings
  • Feelings of hopelessness and/or pessimism
  • Irritability, restlessness, anxiety
  • Feelings of guilt, worthlessness and/or helplessness
  • Loss of interest in activities or hobbies that were once pleasurable
  • Fatigue and decreased energy
  • Difficulty concentrating, remembering details and making decisions
  • Sleep changes.  Either difficulty sleeping or sleeping too much
  • Overeating or appetite loss
  • Thoughts of suicide or suicide attempts
  • Persistent aches or pains, headaches, or digestive problems that do not ease with treatment
If you experience five or more of these symptoms for longer than two weeks or if the symptoms are severe enough to interfere with your daily routine, see a qualified mental health professional for an assessment.
